Leaving hospital in Oxford: a practical checklist

Questions to ask the ward, what to have ready at home, and what to watch for afterwards.

Before discharge

Ask the ward team at the John Radcliffe, Churchill, Nuffield Orthopaedic Centre or Horton General:

  • What therapy will continue at home, and who will provide it?
  • Is any equipment on order, and when is delivery?
  • Is the council's short-term reablement service being arranged?
  • Who should we contact if there are problems after discharge?

On the day

Discharges are often later than planned. Before arrival, make sure:

  • The house is warm and the bed is made up - downstairs if needed
  • There is food and drink in the house
  • Prescriptions have been collected and checked against the discharge letter
  • Walkways are clear of rugs and clutter

The weeks that follow

Encourage regular drinks, small frequent meals and the exercises the physiotherapist set. Contact the GP if there are signs of infection or new confusion. If someone isn't ready to manage alone, short-term live-in care can provide cover until they are.
